     We provide a variety of services to help you at these crucial times when help is so vitally needed. 
 
Services available:

Safe at Home - personal service provided at home. 

Saf “T” Link - personal alarm to get help on it’s way.

Home Care Services
Skilled Care

     Providing a variety of services in the privacy of your home helping to promote recovery. 

     Our team of professionals may include: 

bullet Skilled nursing 
 Physical Therapist 
 Occupation Therapist 
 Speech Therapist 
 Social Worker 
 Dietitian 
 Home Health Aid 

     Services must be ordered by your physician; you must require a skilled care: and if Medicare, you must be considered home-bound to receive these services. 

     Our Agency is Medicare and Medicaid Certified and accepts private insurance.

Franklin County Hospice 
You matter because you are you … till the last moments of your life.”
Dr. Cicely Saunders. 

     A program devoted to loving care, generally provided in the home or the home of a loved one, of those with terminal illness. 

     Hospice offers physical and emotional care to the patient and their family.  Our team approach involves many disciplines coordinating care with the family for the comfort of the dying person. 

     Care provided by a team consisting of:

 Physician 
 Skilled nurse 
 Nursing assistant 
 Volunteers 
 Social Workers 
 Spiritual advisor 
 Dietitian 
 Occupational, physical and/or speech therapists as needed 

Bereavement services provided up to one year following death. 

     Eligibility:

 Terminal Illness 
 Limited life expectancy 
 Cures no longer primary medical goal 
 Loved one or other responsible care giver 
 Consent of physician 

 Developmental Disability Agency
Helping People Help Themselves

     Professional and caring personnel providing opportunities and support for people with developmental disabilities to help each individual achieve their optimal level of independence. 

     Based on individual choices, interests and needs. 

     Completely individualized

 Daily living skills 
 Motor skills 
 Program held in community based settings/natural environment 

     Program will focus on the development of skills which will promote the individuals quality of life. 

     Medicaid eligibility is required. 
 

Personal Care Services
Helping Hands of Certified Aides

     Providing services that will help people remain in their own homes rather than being placed in a nursing home or other institutional care.  This program is for adults, children with physical or developmental disabilities and the elderly. 

     Services provided by Certified Nursing Assistants:

 Assistance with bathing, dressing, grooming 
 Medication assistance 
 Laundry 
 Assistance with exercises 
 Cooking and grocery shopping 
 Housekeeping 
 Transportation to and from medical appointments. 

      Eligibility for Personal Care Service includes:

 Eligible for Medicaid 
 Obtain doctor approval 
 Ability to stay at home safely with intermittent care.

Personal Services Provided at Home

     Services paid privately and the services can be requested by patient or caregiver. 

     Services include:

 Housekeeping 
 Errands 
 Assist with medications 
 Assure safety issues 
All planned to meet your needs. 

LIFELINE
Personal Alarm System

     Can be installed in any home.  One push of a button on your waterproof pendant gets help on its way.  Our monitoring center receives a coded message that allows immediate review of your profile information and takes predetermined steps to dispatch help right away. 

     This service helps you or a loved one to continue to live safely at home, independent and secure.
